This is a regular, full-time (40 hours per week), benefited position.

In this position, our team member will…

  • Provide direct and indirect nursing care to PACE participants, working in close coordination with PACE primary care providers to manage ongoing care and complex clinical situations in the PACE Center and Clinic.
  • Conducts assessments and reassessments and assures Nursing Level of Care documentation is complete.
  • Is an active participant of the Interdisciplinary Team and coordinates accordingly to develop plans of care for PACE participants.

Qualified candidates will have graduated from an accredited school of professional nursing and have an active RN license as well as three years nursing experience with a minimum of one year caring for the elderly or frail.

Employees will have the opportunity to cross-train in multiple food service areas which may include the roles of prep cook, warehouse driver, assistant cook and/or lead cook, depending on experience.
 
In this position, our team member will…

* Be responsible for food handling and packaging of meals in accordance with safety and sanitation procedures
* Clean, wash and prepare food service equipment
* Perform duties such as preparation, service and clean-up of meals
* Maintain and order inventory of janitorial and linen supplies as well as other meal service supplies
* Understand the importance of working as a team, collaborating and working directly with elderly and disabled adults
* Have basic knowledge and understanding of public health, food handling and safety standards as well as a CA Food Handler's card
* Prior experience in food handling, service and clean-up is preferred

• Perform substitute responsibilities for multiple Nutrition Program positions including: lead, assistant or prep cook, food service aide, kitchen aide or van/warehouse driver.
• Be responsible for food handling and service in accordance with food safety guidelines.
• Maintain and track inventory of food and food service supplies.
• Understand the importance of working as a team, collaborating and working directly with elderly and disabled adults.
• Have a high school diploma or GED, basic knowledge of public health food handling/food safety standards, prior experience in food handling and active food safety professional certification; supervisory experience preferred.

The Substitute Dining Center Coordinator has a role of filling in for our team of coordinators as needed and the hours vary depending on program needs (usually ranging from 5.5-6 hours per week). Training is provided for this member of our team to provide coverage at our Arcata, Eureka and Fortuna dining centers.

In this position, team members will...
• Work directly with elderly and disabled adults to support their personal well-being through supervision of food safety guidelines and coordination of nutrition services and activities at our three dining center locations.
• Be responsible for dining center operations including various teams of employees/volunteers and food services at the dining centers and via home delivered meals.
• Participate in training of the dining center team as well as educational programs for participants.
• Learn and understand the importance of working as a team and collaborate to support a continuum of care for our participants.
• Have two years of experience or education in a related field as well as one year of supervisory experience.

Qualified candidates will have a Master's degree from an accredited school of Social Work and two years social work experience. Must have at least one year experience working with the frail or elderly population.
